I was in attendance *I appologize for not dnowing exact dates*, but if memory serves me---sometime between the dates of :late of 1993--mid 1996. DOB-10/19/1969.SS# 257-15-5774. After course/exam..I was employeed with the facility. I took CPR(adult/infant,),&FIrst Aid while in attenance. I am going back to school for additional training in another state--and am requesting any/all info. you may still have archieved on my behalf.
