CAN YOU HELP ME???

I am trying to get a copy of my grandfathers records. I am interested in his admittance records that would contain where he came from and his contact or nearest relative. I would like to know who transferred him there and why.

He was at the San Antonio State Mental Hospital from 1937-1963
Transferred to Austin Confederate Mental Home 1962-
The Austin House closed in 1963 and I lost trace of him from 1963-Sept 1970
Sept 1970 - Dec 20, 1970 (Died) Woodland Heights Hosp Lufkin TX
Woodland Height Hosp death certificate shows you as his residence

I would like something that may bring my grandfather to life for me.

The records I was able to obtain from San Antonio State Hospital show
